Simple Student Fund Raising Order Form  

Simple steps to complete your order

  1. Use ink only to complete all of the boxes. Don't forget your group number.
  2. Include an ITEM NUMBER, NAME, SIZE and price for each item. Order ONE ITEM PER LINE. Note: only those designs pictured on the Kids Korner page are available in CS and XS sizes.
  3. Use the BONUS BOX to select prizes. All prizes except Level X are CUMULATIVE! Prize plan resets after 36 items on a new order form.
  4. Collect money at the time of the order. Checks should made payable to your group.
  5. If you fill out your whole order form, ask your group organizer for a new catalog!
